Treatment was the biggest issue, at least in the state I used to work in. Doctors reported that women were tested but unable to come back for treatment, or were initially treated but their partner wasn’t, and they were then reinfected. I’m a bit rusty on the state by state requirements but ideally women should be tested at the first PNC appointment and again before birth. Not sure if it’s still an issue but several months ago I heard of a national shortage of the specific penicillin that’s best for congenital syphilis, which definitely doesn’t help the situation. *edit – I just finished the article and it looks like the shortage is still happening, but the CDC hasn’t received any reports of pregnant women being unable to access it. I’ve seen guidance encouraging local clinics to prioritize bicillin for pregnant women, and give other forms of penicillin to non-pregnant women when possible. So not a huge issue yet but definitely on the horizon. Have you ever been to a county department of health office? It’s demeaning, cold, and well, just not a very safe space and also, the employees are usually way underpaid. I’m not saying that it isn’t available, but if you’re not showing symptoms of syphilis or any other STD, why bother going to the after thought DMV of healthcare?

To make sure your baby doesn’t get fucking syphilis, that’s a pretty fucking good reason. Even if prenatal care was entirely free, there’s a HUGE demographic that would still not seek appropriate prenatal care. I can tell you this because I read OB ultrasounds on people EVERY DAY who HAVE health insurance and still didn’t get prenatal care until coming into the ED with problem. It’s almost like spending a little bit of public money as prevention prevents a whole slew of very expensive problems later on…. Yep! In my short time in this state, I’ve gotten close enough to a number of women to learn the majority of them just flies out for their “women’s care”. Where are they flying? To blue state doctors. Why? “Oh you know, just stuck in my ways.” Almost rehearsed… I’ve heard that same line so many times-you’d think they all practiced together. They’re FROM HERE. What the hell is their affiliation with these other states? No family there. I internally scream every time I hear it. It’s either that or they just don’t get care. There’s the odd one here and there who says they found a good doctor BUT (and there’s always a ‘but’ of some kind… BUT bring your husband, BUT he’s gonna call you babygirl, BUT blah blah just fml).
