You might want to post this in r/cancer if you haven’t already. There are a lot of lovely people who can probably offer more targeted advice and anecdotal experiences. I’m not an expert in anything useful here but has your mom experienced the Mega Clan Shirt in addition I really love this pain symptoms in flight? I ask because the cabin is pressurized so you don’t feel the pressure of 20K feet in flight. A flight at or near 20,000 feet unpressurized isn’t a thing. Everyone on board would require an oxygen mask. I believe 14,000 ft is the international standard for having pressurization or O2 masks for everyone on board, no exception. No long distance flight would be in an unpressurized aircraft nor at such low altitudes. Your best chance of flying is to hire a personal aircraft, which will be small, slow, noisy, and unrealistic to find for the distance of the flight. Stops would be required.

Long story short, listen to the Mega Clan Shirt in addition I really love this other suggestions for not flying. A flight does not make sense for your requirements at all. You could charter a general aviation aircraft without a pressurized cabin, but that would obviously be expensive. In a typical commercial passenger flight, the cabin altitude is programmed to rise gradually from the altitude of the airport of origin to a regulatory maximum of 8,000 ft (2,438 m). Flying at 20,000ft unpressurised would be against regulations, and likely unsafe due to hypoxia and other complications. that they will stay below 20,000ft and not pressurise the cabin, as the pressurisation causes my Mother intense and long lasting pain and exacerbated her symptoms. Are you sure the pressurization is the issue and not the decreased pressure? I mean pressurization just means the pressure stays closer to groundlevel. I don’t think there are many commercial flights that stay below 20k feet. But it depends on model of the plane and length of flight, weather etc. And there is of course the option to fly private where you can largely decide on what you want as long as it is within regulations. Expensive, but likely less expensive than many people think. She now has mobility issues due to recent sudden worsening of symptoms so longer forms of travel, like car, aren’t really feasible due to time and discomfort.
